Expert Adviser questionnaire
The questionnaire is issued to Expert Advisers who are nominated or ratified by their professional body.
Patient and carer organisation questionnaire
The questionnaire is issued to patient and carer organisations who represent the groups of patients the technology is designed to impact upon.

Sponsor's submission template
This template is completed by the sponsor and contains both the clinical and economic evidence submitted to NICE and the External Assessment Centre.
Patient and carer organisation invitation to comment
This invitation to comment letter is sent to patient and carer organisations who represent the groups of patients the technology is designed to impact upon. It asks them to provide a written statement to NICE to help advise the Committee in developing its guidance.
List of External Assessment Centres
The external assessment centres are commissioned by NICE to provide an independent assessment of the evidence for technologies. The centres below were chosen by public tender and have knowledge of and expertise in the evaluation of medical technologies.

- CEDAR - Cardiff and Vale university health board
- Kings Health Partners
- Newcastle and York consortium - Newcastle upon Tyne hospitals NHS foundation trust
Assessment report
This template is completed by the external assessment centre after reviewing and critically evaluating the sponsor's submission.
