Application process and form
The deadline for applications for Fellowships and Scholarships starting in 2012 is 5pm on 25th November 2011. To be considered for the 2012 intake, applicants are required to complete both of the following steps:
Step 1: Attach the following items to an e-mail and send it to FandSApplications@nice.org.uk, ensuring that the subject field of the e-mail includes your name.
- A completed application form
- A CV
- An institutional letter of reference and organisational support, and
- Two professional letters of reference
The forms available below must be used for all these items (except the CV). Applicants are asked to note that the application form asks for the contact details of all referees and that references will be checked.
Step 2: Print, complete and (where applicable) sign the following:
- an equality monitoring form
- a declaration of interest form
- a disqualification criteria form
and post them, together in one envelope marked ‘confidential,' to:

Please ensure that your name is written legibly on both the declaration of interests form and the disqualification criteria form.
Applications will only be considered if both of the above steps have been taken
