Smoking: support and treatment for people with long term conditions

Indicators

IND154: The percentage of patients with schizophrenia, bipolar affective disorder or other psychoses whose notes record smoking status in the preceding 12 months.

IND155: The percentage of patients with schizophrenia, bipolar affective disorder or other psychoses who are recorded as current smokers who have a record of an offer of support and treatment within the preceding 12 months.

IND156: The percentage of patients with CHD, PAD, stroke or TIA, hypertension, diabetes, COPD, CKD or asthma whose notes record smoking status in the preceding 12 months.

IND157: The percentage of patients with CHD, PAD, stroke or TIA, hypertension, diabetes, COPD, CKD or asthma who are recorded as current smokers who have a record of an offer of support and treatment within the preceding 12 months.

Introduction

This report covers 4 new indicators that are part of the NICE menu of potential Quality and Outcomes Framework (QOF) indicators for 2016/17, following the recommendations of the NICE indicators advisory committee in June 2015.

This report considers the likely resource impact of incentivising the interventions associated with the proposed indicator in terms of the number of additional interventions provided and the cost of each intervention. Costs to NHS commissioners are outlined where relevant.

Number of people affected

The number people eligible for each indicator has not been identified separately as the eligible population is simply moving from one indicator to another.

Current care

The current QOF already incentivises these actions in two existing QOF indicators that cover a wider set of conditions:

SMOK002. The percentage of patients with any or any combination of the following conditions: CHD, PAD, stroke or TIA, hypertension, diabetes, COPD, CKD, asthma, schizophrenia, bipolar affective disorder or other psychoses whose notes record smoking status in the preceding 12 months.

SMOK005. The percentage of patients with any or any combination of the following conditions: CHD, PAD, stroke or TIA, hypertension, diabetes, COPD, CKD, asthma, schizophrenia, bipolar affective disorder or other psychoses who are recorded as current smokers who have a record of an offer of support and treatment within the preceding 12 months.

Proposed care

No changes to clinical care are anticipated as part of these amended indicators.

Resource impact

GPs should currently be asking people with the conditions named in the indicators about their smoking status and offering smoking cessation support and advice under current QOF indicators SMOK002 and SMOK005 as described above. The underlying achievement in 2013/14 was circa 95% for both indicators (Health and Social Care Information Centre, 2014).  

Introducing 4 separate indicators, separating populations with mental illness and other conditions in the 2 existing indicators, is unlikely to result in a resource impact in terms of the provision of clinical care.
